<div xmlns='http://www.w3.org/1999/xhtml'><p class='releaseDateline'>Raleigh, NC (PRWEB) December 26, 2013</p><br /><p>While debate continues among mesothelioma doctors worldwide over which kind of surgery is best, a new review of the medical literature published in Lung Cancer and detailed by Surviving Mesothelioma says the statistics are clearly on the side of the lung-sparing procedure called extended P/D.</p><br /><p>The report reviewed six electronic databases of medical studies and found seven relevant studies comparing lung-sparing extended P/D with lung-removing extrapleural pneumonectomy (EPP) for malignant pleural mesothelioma. According to the study, the mortality rate with EPP is 6.9% while only 2.9% of studied patients die from extended P/D.</p><br /><p>The rate of complications with P/D is also lower, according to the study. Only 27.9% of P/D patients experienced serious complications, compared to more than 60% with EPP. Proponents of EPP, including some of the world’s top mesothelioma doctors, often point to studies showing a survival benefit with EPP. But the new analysis refutes that idea, as well, finding a median overall survival among all the studies of 13-29 months with P/D versus 12-22 months with EPP.</p><br /><p>Extended P/D, the most radical form of P/D, removes the pleural lining and other diseased tissue, but leaves the lung in place. In contrast, EPP involves removal of the entire pleural lining as well as the diseased lung, the lymph nodes, portions of the pericardium around the heart and all or part of the diaphragm. It is typically followed by chemotherapy and/or radiotherapy to kill any residual mesothelioma cells.</p><br /><p>The results of the literature review are likely to reheat the surgery debate among mesothelioma doctors and the authors say they may well change the way the disease is treated. “This may represent an important paradigm shift in the surgical management of malignant pleural mesothelioma,” they write. <div xmlns='http://www.w3.org/1999/xhtml'><p><em>medwireNews</em>: Salvage chest wall resection could lengthen survival in patients with isolated chest wall recurrence of malignant pleural mesothelioma (MPM), research indicates.</p><br /><p>MPM is an aggressive malignancy with a poor prognosis and few effective treatment options, the researchers note. But in their study, 47 patients who underwent chest wall resection for recurrence a median of 16.1 months after initial cytoreductive surgery, the median overall survival was favorable, at 44.9 months.</p><br /><p>Survival was greatest in patients with prolonged time to recurrence and those with the epithelial cell type.</p><br /><p>“[O]ur results have indicated that for select patients with isolated chest wall recurrence of MPM, salvage [chest wall resection] (performed with an intent to cure) is an effective strategy,” say the researchers David Sugarbaker (Harvard Medical School, Boston, Massachusetts, USA) and colleagues.</p><br /><p>The overall study population included 1142 patients who underwent extrapleural pneumonectomy or pleurectomy/decortication. Chest wall recurrence amenable to resection occurred in 47 patients, predominantly in the original thoracotomy incision or in the costophrenic region.</p><br /><p>Most of these patients (68%) had epithelial histology and their time to recurrence was significantly longer than the remaining patients with biphasic histology, at a median of 23.4 months versus 11.2 months. In the former group, younger age was also associated with an increased time to recurrence. Survival following chest wall resection was also longer in patients with epithelial histology, at a median of 20.4 months versus 7.4 months for those with biphasic histology.</p><br /><p>In both groups, survival correlated with time to recurrence. Among those with epithelial histology, the average duration of survival was 8.9 months for those whose chest wall recurrence occurred within 12 months, 17.2 months for those with recurrence at 12 to 23 months, and a considerable 35.8 months for those with recurrence at 24 months or beyond.</p><br /><p>For those with biphasic histology, survival was an additional 15.9 months after chest wall resection for those with recurrence after at least 10 months, but only 2.7 months for those whose time to recurrence was less than 10 months.</p><br /><p>“Thus, this latter group of patients might be better served by palliation or salvage modalities such as chemotherapy or radiotherapy or both,” the researchers suggest in <em>The Journal of Thoracic and Cardiovascular Surgery</em>.</p><br /><p>But for patients with the epithelial cell type, in particular, they conclude: “It is appropriate to consider curative [chest wall resection] as standalone therapy or in conjunction with other adjuvant modalities in the management of localized ipsilateral chest recurrence for patients with a good performance status and for whom the anticipated surgical morbidity is minimal.”</p><br /><p>medwireNews (www.medwirenews.com) is an independent clinical news service provided by Springer Healthcare Limited. © Springer Healthcare Ltd; 2013</p><br /><p>Free abstract</p><br /><img border="0" class="bt_sec" src="http://www.medwirenews.com/lib/comment.gif"/>Tweet</div>

I try to create awareness of its symptoms, its treatments, the amazing breakthroughs that have been made, as well as survival tips and tricks that I have learned through 2 surgeries, 4 chemotherapy drugs, a myriad of side effects (I’m an allergic nightmare!). The Mesothelioma Cancer Alliance has done this and so much more through its website and online community.</p><br /><p>There are so many types of cancer and we all know someone who is fighting this cowardly disease or someone who fought the valiant fight. But I’m not sure if we all know that mesothelioma is a very specific type of cancer – a completely preventable type -- caused only by asbestos exposure. Knowing more about the disease and its risk factors truly does make a difference. By raising more awareness and rallying support, hopefully one day we can get asbestos banned once and for all.</p><br /><h2>Fighting Fires and Asbestos</h2><br /><p>I have a very personal connection to mesothelioma – or at least the cause of it. My husband was a fireman for 20 years in New York City. He worked 24-hour tours in firehouses with asbestos ceilings. Asbestos was used to soundproof the dormitories and other areas of the building. 15 or 20 years after it’s put up, asbestos begins to disintegrate and turns into a powder that floats in the air.</p><br /><p>He spent most of his 20 years running into burning tenement buildings built in the era when entire heating systems and boiler rooms were coated in asbestos. Asbestos was used to insulate and keep the heat in the boilers and the pipes, but, at very high temperatures, as in a fire, it would break down and circulate in the air.</p><br /><p>In the 1970s the city he worked for made a very political statement by cleaning all the asbestos out of the school buildings. It was a very worthwhile beginning, however the firemen in the city were sleeping and eating and living in firehouses with asbestos flaking off the ceilings in the dormitories.</p><br /><p>My husband was an officer with a degree in Fire Science and a Certification in the Occupational Safety Hazard Administration (OSHA). He brought the danger of asbestos to the attention of the City’s administration. Their answer was to paint the ceilings of the affected firehouses. This caused an even greater danger. The paint began to crack and fall, pulling down even more asbestos in the process. When he continued to fight for the firefighters and demand that the asbestos be removed, he was threatened with transfer to a house without asbestos – but with the highest number of fire calls in the City.</p><br /><p>Only recently have I become aware of secondhand exposure to asbestos. The deadly fibers can be transported from one location to another on clothing, shoes, even your body. When I was doing my husband’s laundry, I never dreamed I was exposing myself to a deadly cause of cancer.</p><br /><p>Thank God my husband does not have mesothelioma; however, he does have banding on his lungs and definite damage from years of asbestos exposure. He is monitored constantly for any signs of cancer. Current laws do provide for job-related cancers and the firefighter is entitled to appropriate compensation. The problem stems from the statute of limitations on this law – sixty months. Mesothelioma usually manifests long after the sixty-month statute has expired. It’s more like a ten to thirty year latency period. Various legislators across the country have introduced bills that would extend this statute of limitations for firefighter cancers. Some have been passed and more are still in discussion stages.</p><br /><h2>Awareness and Advocacy Organizations</h2><br /><p>Despite the fact that asbestos exposure is still the number one cause of occupational cancer in the US, the manufacture, importation, processing and distribution in commerce of hundreds of products, are still not banned. The list of EPA Asbestos Related laws and regulations gets longer by the year. Unfortunately, the use of asbestos in products like vinyl floor tile, cement shingles, automatic transmission components, disk brake pads, pipeline wrap, roof coatings, even clothing, is still not banned - and the list goes on and on.</p><br /><p>Roughly 30 million pounds are still used each year. It can still be found in many homes, schools, and commercial or industrial buildings. It was once used in more than 3,000 consumer products including household items such as toasters and hair dryers – some of which may still be in use. Asbestos fibers are invisible to the naked eye and NO AMOUNT of exposure is safe. Navy veterans are at the greatest risk to develop mesothelioma as asbestos was widely used in naval ships and shipyards. But, as with firefighters, many occupations are also at risk.</p><br /><p>My goal is not to frighten anyone, but just to make you aware and to ask for your help, if you are able. There are many organizations that are working tirelessly to eradicate this disease.</p><br /><ul><br /><li>The Mesothelioma Applied Research Foundation, more commonly referred to as MARF, for example, is a non-profit national organization dedicated to lobbying for and funding mesothelioma research initiatives. In addition to funding initiatives and research projects, MARF also seeks to connect patients with cancer specialists and mesothelioma programs at the nation’s leading cancer centers. Another tier of MARF’s mission is advocacy for mesothelioma patients in the form of federal and state-level lobbying efforts.</li><br /><li>The International Mesothelioma Program is a joint initiative of the Brigham and Women’s Hospital, the Dana Farber Cancer Institute and Harvard University School of Medicine. Led by the esteemed Dr. David Sugarbaker, the IMP is at the forefront of all leading mesothelioma treatment research and practice. Combining a holistic treatment and support program, the IMP has made tremendous strides in surgical treatment for early-stage mesothelioma diagnosis as well as aggressive chemotherapy treatments for later-stage disease.</li><br /></ul><br /><p>The National Cancer Institute and the American Cancer Society have always been among the leaders in coordinating research projects. <div xmlns='http://www.w3.org/1999/xhtml'><div><ins data-ad-slot='8194301111' data-ad-client='ca-pub-2565724596955678' class='adsbygoogle c1'> </ins> </div><br /><p>For the first time, researchers from the Perelman School of Medicine at the University of Pennsylvania have shown the predictive power of a group of overlooked lymph nodes–known as the posterior intercostal lymph nodes–that could serve as a better tool to stage and ultimately treat patients with malignant pleural mesothelioma.</p><br /><p>The findings were presented at the 15th World Conference on Lung Cancer.</p><br /><p>Physicians look to lymph nodes to stage essentially all cancers, including mesothelioma. The presence or absence of metastatic cancer cells in lymph nodes affects prognosis and also typically dictates the optimal treatment strategy. But posterior intercostal lymph nodes, which are located between the ribs near the spine, have not been previously used to stage or guide treatment of malignant pleural mesothelioma or any other cancer.</p><br /><p>In a retrospective study of 48 Penn Medicine patients undergoing radical pleurectomy for malignant pleural mesothelioma, Joseph S. Friedberg, MD, Chief of the Section of Thoracic Surgery at Penn Presbyterian Medical Center and Co-Director of the Penn Mesothelioma and Pleural Disease Program, and colleagues found that over half the patients had cancer metastatic to these lymph nodes and that, in some of these patients, those were the only lymph nodes containing metastatic cancer.</p><br /><p>Patients who did not have cancer in the posterior intercostal lymph nodes had significantly longer overall survival rates, nearly two and half years longer, compared to those who did have cancer in the lymph nodes.</p><br /><p>“I am unaware of any other group that is sampling these nodes. They are not currently part of the staging system for mesothelioma, or any other cancer for that matter,” said Dr. Friedberg. “What we have shown here is that even though these lymph nodes are not described in relation to this cancer, they are highly significant.”</p><br /><p>The conclusion of the study was that surgeons should routinely biopsy these lymph nodes as part of any surgery-based treatment for mesothelioma and that these lymph nodes should be included in any revision of the mesothelioma staging system.</p><br /><p>“Ultimately, it means that the presence or absence of cancer in these lymph nodes could help guide the treatment of pleural mesothelioma,” said Dr. Friedberg.</p><br /><p>This study is one of a 13 Penn Medicine studies and talks being presented at the International Association for the Study of Lung Cancer’s 15th World Conference on Lung Cancer.</p><br /><p>###</p><br /><p>Co-authors from Penn Medicine Melissa Culligan, Mary Putt, Stephen M. New Drug Shows Promise in Killing Mesothelioma Tumor Cells - Asbestos.com

<div xmlns='http://www.w3.org/1999/xhtml'><img alt="Chemotherapy needle and tube inserted in patient’s arm" class="article-main-img" itemprop="image" src="http://image.asbestos.com/thumb.php?src=/uploads/news-images/2013/11/chemo.jpg&h=240&w=274" title="This is the post's featured image title attribute."/><br /><p>There may be help on the horizon for future pleural mesothelioma patients needing an effective, first-line systemic chemotherapy regimen that doesn't exist today.</p><br /><p>The latest laboratory breakthrough could give them a much better chance at survival.</p><br /><p>A recently completed study of panobinostat, a new experimental drug, in combination with the standard cisplatin, showed promising results in killing mesothelioma tumor cells without harming the healthy ones, along with eliminating the often troubling side effects.</p><br /><p>The study, published in Genetics and Molecular Research, was conducted in Ankara, Turkey, at Gazi University.</p><br /><p>"None of the currently used standard treatments are acceptable for MPM ( malignant pleural mesothelioma). Studies are needed," professor H. Ilke Onen, Ph.D., Department of Medical Biology & Genetics, Gazi University, told Asbestos.com. "But we found promising results for killing [mesothelioma] tumor cells."</p><br /><h2>Still in Early Stages</h2><br /><p>Onen was quick to point out that his findings in the laboratory were just the start of what would be lengthy process in bringing a potential therapy to market, but the initial research was eye-opening and eventually could move the medical community closer to a more effective treatment.</p><br /><p>Pleural mesothelioma today is typically treated with a FDA-approved chemotherapy combination of cisplatin and pemetrexed, although the response has been far from effective. The majority of mesothelioma patients undergoing treatment do not live beyond 18 months after diagnosis.</p><br /><p>Panobinostat, which is being developed by pharmaceutical giant Novartis, has the ability to stop production of enzymes that allow mesothelioma cancer cells to grow at the molecular level. When the expression of those enzymes is inhibited, the cancer cells are unable to spread. It has shown considerable promise when combined with cisplatin.</p><br /><p>"It's too early to predict an outcome of treatment with that combination [of panobinostat and cisplatin]," Onen said. But they found "an effective cell death response by making cells more susceptible to [cisplatin]."</p><br /><h2>Panobinostat Tested with Other Cancers</h2><br /><p>Panobinostat and cisplatin also are being tested successfully with other cancers, including multiple myeloma and lymphoma.</p><br /><p>The combination of the two drugs, according to test results, was considerably more effective than using each drug separately. Mesothelioma cells were unable to survive in the laboratory while the healthy cells continued to thrive.</p><br /><p>The combination of the two drugs also did not show the same inhibited gene expression, which the authors believe would eliminate the toxic side effects that also comes with typical systemic chemotherapy. It would allow for a much stronger dose.</p><br /><p>"The result is a more effective removal of tumor cells," the authors wrote in the article.</p><br /><p>Despite the impressive study results, Onen said the laboratory results were only the first step. His research group already has moved into the second step that would include animal models. NGR-hTNF, a novel vascular targeting agent, could provide an answer for a pathology currently lacking any approved therapy for first line maintenance and second line therapy.<br/><br />The study, already ongoing in Europe, foresees the enrolment of 100 patients in Europe and Russia by the end of 2014.</p><br /><strong>About pleural mesothelioma</strong><br /><p>Pleural mesothelioma is a form of cancer strongly related with repeated exposure to asbestos fibres (for more information please refer to Park et al, Global Magnitude of Reported and Unreported Mesothelioma, Environmental Health Perspectives, 2011). With an incidence of approximately 1/100,000, pleural mesothelioma is still a relatively rare type of cancer, but has been progressing quickly in the past 20 years as incidence rates have continuously increased. Pleural mesothelioma has a long latency period and symptoms are non-specific, so that in most cases diagnosis is difficult before the advanced stage of the disease is reached. Research indicates that pleural mesothelioma may affect each year more than 10,000 people in the EU and the US. Currently, only one therapy has been approved for first line treatment, whilst the elevated need for first line maintenance and second line treatment remains unmet. Clinical development of NGR- hTNF - currently in Phase III for second line treatment and in Phase II in first line maintenance <span>-</span> is aimed at filling this therapeutic gap.</p><br /><strong>About NGR-hTNF</strong><br /><p>NGR-hTNF is a novel therapeutic agent for solid tumours which displays antitumor activity through its specific binding to blood vessels feeding the tumour mass. He was 87.</p><br /><p>The way the Country Music Hall of Fame member fought cancer was an apt metaphor for the way he lived his life, always fiercely charting a path few others might have the fortitude to follow.</p><br /><p>Along the way he changed the sound of country music, collaborated with and inspired the genre's biggest stars and remained relevant for more than half a century.</p><br /><p>"Ray Price was a giant in Texas and country western music. Besides one of the greatest voices that ever sang a note, Ray's career spanned over 65 years in a business where 25 years would be amazing," said Ray Benson of the country music group Asleep at the Wheel.</p><br /><p>Price, one of country music's most popular and influential singers and bandleaders, had more than 100 hits and was one of the last living connections to Hank Williams.</p><br /><p>Country Music Hall of Fame and Museum historian Michael McCall said Price "was one of his generation's most important musical innovators," popularizing the bedrock 4/4 shuffle beat that can still be heard on every honky-tonk jukebox and most country radio stations in the world.</p><br /><p>"His emphasis on the shuffle rhythm influenced every generation to follow and remains a staple of country dance floors everywhere, especially in the Southwest," said McCall.</p><br /><p>Price died Monday afternoon at his ranch outside Mount Pleasant, Texas, said Billy Mack Jr., who was acting as a family spokesman. Billie Perryman, the wife of family friend and spokesman Tom Perryman, a DJ with KKUS-FM in Tyler, also confirmed his death.</p><br /><p>Price's cancer had recently spread to his liver, intestines and lungs, according East Texas Medical Center in Tyler. He stopped aggressive treatments and left the hospital last Thursday to receive hospice care at home.</p><br /><p>At the time, his wife, Janie Price, relayed what she called her husband's "final message" to his fans: "I love my fans and have devoted my life to reaching out to them. I appreciate their support all these years, and I hope I haven't let them down. I am at peace. I love Jesus. I'm going to be just fine. Don't worry about me. I'll see you again one day."</p><br /><p>Perhaps best known for his version of the Kris Kristofferson song "For the Good Times," a pop hit in 1970, the velvet-voiced Price was a giant among traditional country performers in the 1950s, '60s and '70s, as likely to defy a trend as he was to defend one. He helped invent the genre's honky-tonk sound early in his career, then took it in a more polished direction.</p><br /><p>He reached the Billboard Hot 100 eight times from 1958-73 and had seven No. 1 hits and more than 100 titles on the Billboard country chart from 1952 to 1989. "For the Good Times" was his biggest crossover hit, reaching No. 11 on the Billboard pop music singles chart. His other country hits included "Crazy Arms," ''Release Me," ''The Same Old Me," ''Heartaches by the Number," ''City Lights" and "Too Young to Die."</p><br /><p>Price was elected to the Country Music Hall of Fame in 1996, long after he had become dissatisfied with Nashville and returned to his home state of Texas.</p><br /><p>His importance went well beyond hit singles. He was among the pioneers who popularized electric instruments and drums in country music. After helping establish the 4/4 shuffle in country music, Price angered traditionalists by breaking away from country. He gave early breaks to Nelson, Roger Miller and other major performers.</p><br /><p>His "Danny Boy" in the late 1960s was a heavily orchestrated version that crossed over to the pop charts. He then started touring with a string-laden 20-piece band that outraged his dancehall fans.</p><br /><p>In the 1970s he sang often with symphony orchestras — in a tuxedo and cowboy boots.</p><br /><p>Like Nelson, his good friend and contemporary, Price simply didn't care what others thought and pursued the chance to make his music the way he wanted to.</p><br /><p>"I have fought prejudice since I got in country music and I will continue to fight it," he told The Associated Press in 1981. "A lot of people want to keep country music in the minority of people. But it belongs to the world. It's art."</p><br /><p>In the same 1981 interview, he credited the cowboy for the popularity of country music.</p><br /><p>"Everyone loves the cowboy. He's nice, humble and straightforward. And country music is the same thing. The kids have discovered what mom and pop told 'em."</p><br /><p>Price continued performing and recording well into his 70s.</p><br /><p>"I have to be in the business at least five or 10 more years," Price said in 2000, when he and his band were doing 100 shows a year.</p><br /><p>"Two or three years ago, we did 182," he said. "Fans come to the shows, bless their hearts, they always come."</p><br /><p>In 2007, he joined Haggard and Nelson on a double-CD set, "Last of the Breed." The trio performed on tour with the Texas swing band Asleep at the Wheel.</p><br /><p>"I'll be surprised if we don't all get locked up somewhere," Price joked at the time.</p><br /><p>Over the years, Price came in and out of vogue as traditional country music waxed and waned on the radio. He was a constant advocate for the old days and ways of country music, and more recently re-entered the news when he took offense to comments Blake Shelton made about classic country music that included the words "old farts." The dustup drew attention on the Internet and introduced Price to a new generation of country fans.</p><br /><p>"You should be so lucky as us old-timers," Price said in a happily cantankerous post in all capital letters. "Check back in 63 years (the year 2075) and let us know how your name and your music will be remembered."</p><br /><p>Price earned his long-standing fame honestly, weaving himself into the story of modern country music in several ways.</p><br /><p>As a young man, Price became friends with Williams, toured with the country legend and shared a house with him in Nashville. Williams even let Price use his band, the Drifting Cowboys, and the two wrote a song together, the modest Price hit "Weary Blues (From Waiting)".</p><br /><p>By 1952 Price was a regular member of the Grand Ole Opry.</p><br /><p>The singer had one of country music's great bands, the Cherokee Cowboys, early in his career. His lineup included at times Nelson, Miller and Johnny Paycheck.</p><br /><p>His 1956 version of "Crazy Arms" became a landmark song for both Price and country music. His first No. 1 country hit, the song rode a propulsive beat into the pop top 100 as well. Using a drummer and bassist to create a country shuffle rhythm, he eventually established a sound that would become a trademark.</p><br /><p>"It was strictly country and it went pop," Price said of the song. "I never have figured that one out yet."</p><br /><p>Price was born near Perryville, Texas, in 1926 and was raised in Dallas. He joined the Marines for World War II and then studied to be a veterinarian at North Texas Agricultural College before he decided on music as a career.</p><br /><p>Soft-spoken and urbane, Price told the AP in 1976: "I'm my own worst critic. I don't like to hear myself sing or see myself on television. <div xmlns='http://www.w3.org/1999/xhtml'><p>By Lucy Piper, Senior medwireNews Reporter</p><br /><p>Salvage chest wall resection could lengthen survival in patients with isolated chest wall recurrence of malignant pleural mesothelioma (MPM), research indicates.</p><br /><p>MPM is an aggressive malignancy with a poor prognosis and few effective treatment options, the researchers note. But in their study, 47 patients who underwent chest wall resection for recurrence a median of 16.1 months after initial cytoreductive surgery, the median overall survival was favorable, at 44.9 months.</p><br /><p>Survival was greatest in patients with prolonged time to recurrence and those with the epithelial cell type.</p><br /><p>“[O]ur results have indicated that for select patients with isolated chest wall recurrence of MPM, salvage [chest wall resection] (performed with an intent to cure) is an effective strategy,” say the researchers David Sugarbaker (Harvard Medical School, Boston, Massachusetts, USA) and colleagues.</p><br /><p>The overall study population included 1142 patients who underwent extrapleural pneumonectomy or pleurectomy/decortication. Chest wall recurrence amenable to resection occurred in 47 patients, predominantly in the original thoracotomy incision or in the costophrenic region.</p><br /><p>Most of these patients (68%) had epithelial histology and their time to recurrence was significantly longer than the remaining patients with biphasic histology, at a median of 23.4 months versus 11.2 months. In the former group, younger age was also associated with an increased time to recurrence. Survival following chest wall resection was also longer in patients with epithelial histology, at a median of 20.4 months versus 7.4 months for those with biphasic histology.</p><br /><p>In both groups, survival correlated with time to recurrence. Among those with epithelial histology, the average duration of survival was 8.9 months for those whose chest wall recurrence occurred within 12 months, 17.2 months for those with recurrence at 12 to 23 months, and a considerable 35.8 months for those with recurrence at 24 months or beyond.</p><br /><p>For those with biphasic histology, survival was an additional 15.9 months after chest wall resection for those with recurrence after at least 10 months, but only 2.7 months for those whose time to recurrence was less than 10 months.</p><br /><p>“Thus, this latter group of patients might be better served by palliation or salvage modalities such as chemotherapy or radiotherapy or both,” the researchers suggest in <em>The Journal of Thoracic and Cardiovascular Surgery</em>.</p><br /><p>But for patients with the epithelial cell type, in particular, they conclude: “It is appropriate to consider curative [chest wall resection] as standalone therapy or in conjunction with other adjuvant modalities in the management of localized ipsilateral chest recurrence for patients with a good performance status and for whom the anticipated surgical morbidity is minimal.”</p><br /><p class='springer-copy'>Licensed from medwireNews with permission from Springer Healthcare Ltd. ©Springer Healthcare Ltd. Pemetrexed (Alimta) is the primary first-line chemotherapy drug for mesothelioma patients with unresectable disease. The drug is usually paired with a platinum-based agent like cisplatin.</p><br /><p>But there is little information in the medical literature about what to do when first-line mesothelioma treatment does not work. Vinorelbine, a drug derived from the rosy periwinkle and marketed under the brand name Navelbine, may be a safe and effective choice, according to a new study published in the journal Lung Cancer.</p><br /><p>Fifty-nine mesothelioma patients were included in the retrospective Italian study. Their median age was 69 and all had received at least one round of pemetrexed-based chemotherapy. Patients were given 25mg/m2 of vinorelbine intravenously every three weeks for a maximum of 6 cycles. Treatment stopped when a mesothelioma tumor began growing again or when the side effects became too severe.</p><br /><p>The best news is that almost half of the mesothelioma patients in the study (49.1%) saw either partial response or stabilized disease as a result of vinorelbine treatment. Overall survival was 6.2 months. Patients who had the best response were those who had a high ECOG performance status, a measure of cancer patients’ general well-being, and those who had experienced at least six months of progression-free survival after their first-line chemotherapy.</p><br /><p>Just as importantly, vinorelbine was well-tolerated by most of the mesothelioma patients. Only five patients experienced grade 3 or 4 neutropenia, a sharp decline in the white blood cells called neutrophils. For most patients, the most bothersome effects of vinorelbine treatment were fatigue and constipation.</p><br /><p>“Vinorelbine was moderately active in pemetrexed-pretreated malignant pleural mesothelioma patients with an acceptable toxicity profile,” the researchers conclude in the journal Lung Cancer. Although the primary outcome for the trial was to measure survival at one year after treatment, researchers report that while survival rate did not improve, quality of life did.</p><br /><p>According to an article in Oncology Practice, the partial pleurectomy <strong>significantly improved</strong> quality of life and control of pleural effusion compared with talc pleurodesis. Both treatments are used for pleural effusion. Up to this point, no studies have shown which therapy is more effective in treating pleural effusion caused by malignant mesothelioma, but pleurectomy is often favored.</p><br /><p>Pleural effusion is an accumulation of fluid between the layers of tissue that line the lungs and chest cavity. In healthy adults the body produces pleural fluid in small amounts to lubricate the surfaces of the pleura. This fluid is normally removed by lymphatics in the parietal pleura, which have the capacity to absorb 20 times more fluid than is normally formed. When this capacity is overwhelmed, a pleural effusion develops. Pleural effusion is found in 95% of all pleural mesothelioma cases.</p><br /><p>The MesoVATS trial, conducted by Papworth Hospital NHS Foundation Trust of the UK, included 175 patients with malignant pleural mesothelioma and pleural effusion. Half of the patients received a partial pleurectomy, and the other half were treated with talc pleurodesis with slurry or poudrage. While the survival rates were similar for the patients (57% in talc patients, 52% in pleurectomy), the differences in the control of the pleural effusion were significant. The effusion was controlled in one month in 59% of the pleurectomy group, but in just 37% of the pleurodesis group.</p><br /><p>“Quality of life in those last 12-13 months of life is very important, and [surgery] is something that is fairly well tolerated and doesn’t require a lot longer hospital stay,” said Dr. Robert Rintoul, lead researcher and consultant respiratory physician at Papworth Hospital, Cambridge, England.</p><br /><p>Pleural mesothelioma is an asbestos-caused pulmonary cancer found in the outer lining of the lungs called the mesothelium. Mesothelioma patients typically suffer from pleural effusion that can seriously restrict the lung’s ability to expand, even collapsing its supportive tissues, resulting in pain and shortness of breath.
